- Length of time lived in the UK
- Home Office reference number
- Travel document or passport (if one is held)
- Have you been out of the UK for more than 6 continuous months since been given leave to enter of remain?
- Current National Insurance Number
- What type of enter of Leave to remain status you have
- Your BRP number, if known
- (Copy of BRP)
- Information about how and where you lost or had your BRP stolen
- You must have reported this to the police and have a crime reference number if one was given
- You must have reported the loss or theft online to the Home Office
- You must print off and sign the consent form giving permission for the Home office to request verification checks
- Evidence for proof of address
